Some implementations of video playback produce audio that circumvents Tab Mute.

I know this is reproducible here, though it requires sign-in from any cable tv provider: The broadcast stream at http://www.msnbc.com/now

Before the stream, some ads are delivered through an overlaid secondary player (possibly not Flash?) -- the audio from these > does not cause Audio icon to appear. > Mute Tab (via context menu) does NOT mute this audio.

Once the ad is over and the network stream is playing, the icon's appearance and the Mute Tab function work as expected.
