Make sure that you do not run Firefox in full screen mode (press F11 or Fn + F11 to toggle; Mac: command+Shift+F).
If you are in full screen mode then hover the mouse to the top to make the Navigation Toolbar and Tab bar appear.
You can click the Maximize button at the top right to leave full screen mode or right click empty space on a toolbar and use "Exit Full Screen Mode" or press F11.

If the menu bar is hidden then press the F10 key or hold down the Alt key, that should make the menu bar appear.

Make sure that toolbars like the "Navigation Toolbar" and the "Bookmarks Toolbar" are visible: "View > Toolbars"

  • If items are missing then open the Customize window via "View > Toolbars > Customize" or via "Firefox > Options > Toolbar Layout"
  • If a missing item is in the toolbar palette then drag it back from the Customize window on the toolbar
  • If you do not see an item on a toolbar and in the toolbar palette then click the "Restore Default Set" button to restore the default toolbar set up.

This feature sucks, and the "fix" is useless. I want a permanent fix, not a work around.

If you have the 6.0 version, you should see the orange "Firefox" button, upper LEFT corner. In any case, right click somewhere at the top of your screen. "Check" the "Menu bar" and the orange "Firefox" button will disappear, while you'll get the familiar File, Edit, View etc Menu Bar! Very easy fix, if you know how...shame on you Mozilla for complicating things for no apparent reason!
