after upgrading to FF4, my back, forward, buttons no longer work

After I upgraded to FF4, my back, forward, and refresh buttons no longer work. I have tried restarting with Addons disabled, still an issue. Even the Alt-back arrow does not work. As well, the URLs no longer appear in the address box when I use Most Visited shortcuts.

duh, I thought that I had disabled all the addons, but I hadn't. The culprit turned out to be: Favicon Picker 2 0.6.1.14.

After disabling this add-on, everything went back to normal.
