How do I add an item from a menu (File, Edit,.......Tools, Help) to the toolbar? "The Customize Toolbar" has only a limited/fixed number of items that can be added to the Mail Toolbar. Not each item from each menu appears in the "Customize Toolbar" window.

I suggest you look at the Toolbar Buttons add-on. I glanced through the (long) description and I think it may offer what you want -- worth a look anyway.
