Hello,
Firefox's safe mode turns off all extensions to troubleshoot problems, and you can't use text to speech in this mode.
Firstly, check if the extension is allowed to run in different browsing modes. For example, allowing the extension to run in private mode.
Next, make sure that volume is also not disabled.
Lastly, you can try disable ehanced tracking protection.
