Open in a new window is no longer usable in v4.0.1

After installing v4.0.1 the open in a new window function no longer works properly. A new window is opened, but where the bookmarks toolbar should be displayed it shows a blank line. The navigation toolbar is displayed, but does not function. If I click on links in the displayed page the URL in the navigation toolbar does not change to reflect the new page I am seeing. The reload button in the navigation toolbar will not reload the page.

Start Firefox in Use Troubleshoot Mode in Firefox to check if one of the extensions is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).
