insert menu is gone. The bar only has "send" "spelling" "attach" "security" and "save".
I used to be able to insert an image under "insert", but now that choice is missing from the top bar. Under "attach, my only choices are: file(s), web page, personal card (vcard) and "remind me later". I presume this was after an "upgrade"!!

I think you have switched to plain text email, where you can attach, but not insert graphics.
Open the accounts settings and see if the checkbox for "Compose in html format" is ticked.

I think settings spontaneously change, especially when Thunderbird gets updated.
