I just installed Firefox 4, and the back and forward buttons are greyed out. Also I am no longer able to refresh a page.

This can be a problem with the file places.sqlite that stores the bookmarks and the history.
I just followed those instructions (deleting the "places.sqlite" file from the profiles folder, but still, nothing!
Any other ideas?
I had that problem. If you're running McAfee SiteAdvisor, disable it and restart Firefox.
McAfee is working on a fix, according to their support forum.
