it is not remembering login and passwords I tell it too; no exceptions are set

I have many websites that I use logins for and I have told it to remember them. I went to a website today that it new my login but not my password. I have entered it at least 10 times now. I went in to Options to view passwords and there were only 3 even in the file. The do you want to remember this login & password question did not come up. There are no exceptions set so I do not understand what is going on.
