Why do the add-on icons change position or disappear completely?

I will customize the add-on icons on the toolbars (3) but when I close FF 37.0.2 and reopen it the icons are in different places or gone completely. One of the reasons I love Firefox is all the add-ons enable me to do so many things so easily. I spend several hours each day on the net working many different projects and trying to do what I do without the add-ons easily accessible things are more difficult. Please advise.

Please attach some screenshots demonstrating the problem you are experiencing (one of Firefox with the toolbar customizations made, and another of Firefox after it is restarted and has forgotten the changes).
