Why will Firefox not open a form from the existing web page
Using Firefox, I go to a web page, and on it, there is a button to click for a contest entry. Clicking the button should open a form (not a new web page) to fill out. Nothing happens. This has worked for me in the past, I have entered this contest daily up until last Friday (11/22). Saturday (11/23), this would not work.
I have contacted the web site, and they claim everything is working fine, and they have tried this on multiple web browsers, with no problems - it works properly.

Sounds that you are either blocking the JavaScript $('.col').addClass('phase2'); to enter phase2 or there is a problem with the CSS that would make that form visible once you have clicked the button to enter phase2.
It is weird that it happens in other browsers as well and that booting the computer in Safe Mode doesn't have effect.
You can try to reset (power off/on) the router.
You can remove all data stored in Firefox from a specific domain via "Forget About This Site" in the right-click context menu of an history entry ("History > Show All History" or "View > Sidebar > History") or via the about:permissions page.
Using "Forget About This Site" will remove all data stored in Firefox from that domain like bookmarks, cookies, passwords, cache, history, and exceptions, so be cautious and if you have a password or other data from that domain that you do not want to lose then make a note of those passwords and bookmarks.
You can't recover from this 'forget' unless you have a backup of the involved files.
It doesn't have any lasting effect, so if you revisit such a 'forgotten' website then data from that website will be saved once again.
Do a malware check with some malware scanning programs on the Windows computer.
Please scan with all programs because each program detects different malware.
All these programs have free versions.
Make sure that you update each program to get the latest version of their databases before doing a scan.

Boot the computer in Windows Safe Mode with network support (press F8 on the boot screen) as a test to see if that helps.
You can try these steps in case of issues with web pages:
Reload web page(s) and bypass the cache to refresh possibly outdated or corrupted files.
- Hold down the Shift key and left-click the Reload button
- Press "Ctrl + F5" or press "Ctrl + Shift + R" (Windows,Linux)
- Press "Command + Shift + R" (Mac)
Clear the cache and cookies only from websites that cause problems.
"Clear the Cache":
- Firefox/Tools > Options > Advanced > Network > Cached Web Content: "Clear Now"
"Remove Cookies" from sites causing problems:
- Firefox/Tools > Options > Privacy > Cookies: "Show Cookies"
Start Firefox in Safe Mode to check if one of the extensions (Firefox/Firefox/Tools > Add-ons > Extensions) or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox/Firefox/Tools > Add-ons > Appearance).
- Do NOT click the Reset button on the Safe Mode start window.
cor-el, Thanks for the tips and suggestions. Unfortunately, I tried each, and none of them worked.
Here is more information about the problem. 1. The web site that I tried to get to is this: http://sportsillustrated.cnn.com/sweepstakes/SI/bp/. This loads OK. 2. Once at this site, you must click the "Enter the Sweepstakes" button ( I am already a member). This is the issue - the form that should appear does not, nothing happens, and this has worked for me in the past. 3. It is a contest with BP and Sports Illustrated that can be entered daily. I have entered it successfully, daily, up until 11/23. 4. Since then, I have not been able to load the form that is used for entry. 5. I have used Firefox to do this, but have now tried IE and Chrome, and each of those browsers have the same results - the entry form does not load. 6. This is not so much an issue with entering the contest, but trying to find out what issue I have with my PC and browser, for any other similar web pages.
Any help is greatly appreciated! Jim

That was the issue. I did a search for the InternetCorkboard.com virus, and then learned that there were several types of virus that could launch it.
