Why do Firefox windows no longer shadow the program icon in the task bar?

Icons of windows using the same program were previously grouped together in the task bar as I've selected "Always combine, hide lables" in my task bar menu.

However, since updating Firefox this weekend, this no longer happens with the Firefox icons, although it does still with my Outlook and Word windows. See screenshot with notes. Any help appreciated. Thanks.
