Like the title says, it creates multiple volume tabs in the Volume Mixer. I have my main volume sound set to 100% but set firefox at a lower volume and whenever a new tab opens that has audio in it, the new volume knob defaults to 100% thus blowing my ears out with extremely loud sound.

I'm using Firefox 56.0 64 bit. I have already tried refreshing, uninstalling and reinstalling.

There is a Bug filed about each additional Firefox process that is running creating an additional Volume Mixer control AND not respecting the initial process' user set Volume level. IOW, a Bug in Firefox 56.
