Are you possibly using permanent Private Browsing mode (Always use Private Browsing mode; Never Remember History) ? In PB mode the disk cache isn't used, but instead an in memory cache is used that is purged automatically when you close Firefox. This also means that you can't clear the disk cache as long as you are in permanent PB mode, so you need to restart Firefox in regular mode.
