The proxy moz-proxy://proxy.xxxxxxxxx.gov:8080 is requesting a username and password. The site says: "moz-proxy://proxy.xxxxxxxx.gov:8080"

I get this pop-up ALL the time on only one certain site and it even switches tabs when it pops-up which is very very annoying

Check the connection settings.

If you do not need to use a proxy to connect to internet then try to select "No Proxy" if the "Use the system proxy settings" setting or one of the others doesn't work properly.
