I'm randomly getting a Header/banner injected into into Web pages while browsing. The banner is stuck to the top of the web page allowing the requested page scroll below/beneath. The phishing attempt is requesting I fill out an infrastructure questionnaire (Account holders, Modem Type and Serial number, router type and serial number, etc.) as if it were from my ISP. There are YES/NO buttons to click. They link to an in page link to the originally requested page.

Antivirus Software: ClamXAV Browser: Firefox ISP: Optimum last injection: https://www.ledostilwellroad.com/ The two Yes/No buttons like to: https://www.ledostilwellroad.com/# (Note # at end of URL) Antivirus scan found:

File Name Infection Name Status Current Path Original PathGoodGame Empire.app PUA.OSX.AMC Trashed /Users/gmandas/.Trash/GoodGame Empire.app /Applications/GoodGame Empire.app

My ISP said, "contact your antivirus company, see ya' later."

Is this a know phishing scam? What should I do? gmandas@yahoo.com
