Headers/Banners injected into Web pages
I'm randomly getting a Header/banner injected into into Web pages while browsing. The banner is stuck to the top of the web page allowing the requested page scroll below/beneath. The phishing attempt is requesting I fill out an infrastructure questionnaire (Account holders, Modem Type and Serial number, router type and serial number, etc.) as if it were from my ISP. There are YES/NO buttons to click. They link to an in page link to the originally requested page.

You can do a malware scan on your Mac.
