That doesn't ring a bell. Do you want to post a screenshot of it? Please cut out or blur any sensitive information in the image before attaching it to your reply.
Also, your Firefox identified as "Mozilla/5.0 Netscape Firefox/40.99 da Els/3.0.6" which doesn't sound familiar. Is it a custom build?
