Why are the Awesome Bar suggestions disappearing?
Anytime a suggest box pops up, or the menu for Firefox it is immediately hidden. The outline for the box remains and if you mouse over the box where the text should be it will flash for a second and then disappear again.
I've tried disabling all add-ons and extensions but the problem persists.

This issue can be caused by an extension or theme that isn't working properly.
Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the add-ons is causing the problem (switch to the DEFAULT theme: Tools > Add-ons > Themes).
- Don't make any changes on the Safe mode start window.

As I said in my initial post "I've tried disabling all add-ons and extensions but the problem persists."
The only difference I see between safe mode and manually disabling is the Default 4.0 theme is disabled in Safe Mode. You cannot disable this manually. When the default theme is disabled, things work.
Also, there is no Tools menu in 4.0.
Turning off "Use hardware acceleration when available" through
Firefox button > Options > Options > General tab
solved the issue.
