Typical problems include BACK/FORWARD and what middle-click do. Firefox developers tend to mess with user settings from time to time. You will find ":FORWARD" under Back in your choices (not strictly alphabetical). I have "Enter" for middle-click and the (fill-in-the-blank) seem to think I should have something different.
Check out what you have in the control panel for "Mouse".

Ntrust mouse, it doesn't seem to want to work at the top quarter of the screen, works fine lower down the page, but this only happens when using firefox, all is ok when using google chrome.
