I am having a problem with customized controls reverting to default

I have been having a problem for a while I have rearranged the controls in the Firefox Navigation toolbar. It is very sporadic and has been difficult to track down. Recently I decided it was due to Tab Mix Plus, but last night it happened again. This time I happened to notice something.

I open FireFox often by clicking forum links in emails. Sometimes this results in Firefox opening 2 windows (windows not tabs). Last night I noticed that two windows opened. The first one displayed the icons properly. The second one opened with the default interface. When I closed both windows, but then I opened up Firefox again it had the default layout.

Does anyone have any idea's why this happens, and if there is something I can do to fix it?

I have seen this. Sometimes when another window is opened, the icons get mixed up. Then when you close down Firefox, that last window set up is what gets saved.
