Why does 5.0.1 play the same audio & video files sometimes and not others?

I have links to media files with suffixes like aif, mp3, mov. The files often play the first time, but won't play again. Other times they won't play at all. Using 5.0.1 which should cause QuickTime plugin (latest installed) to run. It is simply inconsistent with the playing being more rare than not playing.
