I no longer have a Star Icon in my Locator Bar

I'm running FF 9.0.1 on Windows 7.

The Star Icon is missing from the Locator Bar & if I bookmark a site, either via ^D or "Bookmark this Page" it does not bring up the window where I can choose where to place my bookmark. Instead it just placed the bookmark at the end of the list of bookmarks.
