It depends...

The "Always ask you where to save files" is linked to the "Save File" option. If the action set for the particular file type is "Save File" (most of them, except PDFs), then Firefox should ask you where to save the file before saving it.

But if the action is set to something else, then the behavior varies:

"Always Ask" - Firefox shows a dialog where you can choose between an "Open with" option (auto-saves then opens) and a "Save File" option (asks you where to save)

"Use [relevant application]" - Firefox autosaves and opens

"Open in Firefox" (for PDFs) - Firefox autosaves and opens, but where Firefox saves -- web content cache or a regular folder -- varies depending on whether the server tries to force a download
