Firefox keeps making new book marks called "new tab", how can i stop this?

i clear this every once in a while but i always have multiple "new tab" bookmarks. Am i possibly pressing some keyboard shortcut which causes this?

How are you clearing the history, via built-in means or external cleanup software ?

When do those new tabs appear? Is that when you open Firefox or randomly or possibly when you download a file ?
