Downloads set to save file but it still prompts to ask

I have my downloads set to to save file but a save prompt still pops asking what where etc to save.

I have redundantly troobleshooted start over old new, uninstall re install etc but nothing seems to fix it.

Check the save action for each of the individual file types and for "other files" below that.
