Firefox crashes after inserting smart card
Here is the Firefox error message:

- Enter about:crashes in the Firefox address bar and press Enter. A Submitted Crash Reports list will appear, similar to the one shown below.
- Copy the 5 most recent Report IDs that start with bp- and then go back to your forum question and paste those IDs into the "Post a Reply" box.
Note: If a recent Report ID does not start with bp- click on it to submit the report.

This module is triggering 3 of those crashes - lpxpcom.dll = Last Pass
Two DLL's, one from TortoiseSVN, and another from ActivClient Mini from ActivIdentity may have contributed to 2 of those crashes.
Is Last Pass up-to-date?

So, is Last Pass up-to-date? Have you tried disabling LastPass to see if that crashing stops? How about the other two applications that I mentioned?
The most recent crash report is from an older Firefox 38.6.1esr version (current is 38.7.1esr).
Can you try to update Firefox?
Start Firefox in Safe Mode to check if one of the extensions ("3-bar" menu button or Tools > Add-ons > Extensions) or if hardware acceleration is causing the problem.
- Switch to the DEFAULT theme: "3-bar" menu button or Tools > Add-ons > Appearance
- Do NOT click the "Refresh Firefox" button on the Safe Mode start window
