Does this occur when certain events happen, such as:
- Exit Firefox and start it back up again
- Shut down Windows and start it back up again
or it it more random, or does Firefox never use your default search engine?

It happens when I exit Firefox and restart it. I tried the fixc preferences article but it did not help.
There is software like Advanced SystemCare with its Surfing Protection feature that can protect files in the Firefox profile folder against changes. If you have such software then check the settings or uninstall this software.
