Hi hiral2cool,

That is very strange. I think this has to do with the ico file being enabled or disabled in about:config ? Try to disable javascript and restart the browser. You can still access it from about:config or by installing an add-on.

about:config

  1. In the address bar, type "about:config" (with no quotes), and press Enter.
  2. Click "I'll be careful, I promise"
  3. In the search bar, search for "javascript.enabled" (with no quotes).
  4. Right click the result named "javascript.enabled" and click "Toggle". JavaScript is now disabled.

To Re-enable JavaScript, repeat these steps.

Add-ons

You can alternatively install an add-on that lets you disable JavaScript, such as

  • No-Script (to disable JavaScript on a per page basis, as required)
  • QuickJava (to easily disable and enable JavaScript, automatic loading of images, and other content)
  • SettingSanity (adds back removed options for JavaScript, Advanced JavaScript Settings, loading images, and showing the Tab Bar)

Thank you and I hope this helps!

Could be this CSS rule in chrome://browser/content/tabbrowser.css

.tab-icon-image:not([src]):not([pinned]),
.tab-throbber:not([busy]),
.tab-throbber[busy] + .tab-icon-image {
  display: none;
}

Add code to the userChrome.css file below the default @namespace line.

@namespace url("http://www.mozilla.org/keymaster/gatekeeper/there.is.only.xul"); /* only needed once */

.tab-icon-image:not([src]):not([pinned]) { display: -moz-box !important; }

The customization files userChrome.css (user interface) and userContent.css (websites) are located in the chrome folder in the Firefox profile folder.

Firefox uses the chrome protocol to access internal files and also files from installed extensions.
You can paste such a link in the Firefox location bar to inspect such a file.

  • Create the chrome folder (lowercase) in the <xxxxxxxx>.default profile folder if this folder doesn't exist.
  • Use a plain text editor like Notepad to create a (new) userChrome.css file in this folder (the names are case sensitive).
  • Paste the code in the userChrome.css file in the editor window and make sure that the userChrome.css file starts with the default @namespace line.
  • Make sure that you select "All files" and not "Text files" when you save the file via "Save file as" in the text editor as userChrome.css.

Otherwise Windows may add a hidden .txt file extension and you end up with a not working userChrome.css.txt file.

You can use this button to go to the currently used Firefox profile folder:

  • Help > Troubleshooting Information > Profile Directory: Show Folder (Linux: Open Directory; Mac: Show in Finder)

Those pages do not have an icon as the src is missing, so you get the default icon and that is an empty box. specify a new default icon as well

I have code like this to cover more cases:

#tabbrowser-tabs .tab-icon-image:not(src),
.sidebar-placesTreechildren::-moz-tree-image(leaf),
.bookmark-item:not(src):not([container]):not([type]){
 list-style-image: url("chrome://global/skin/icons/folder-item.png")!important;
 -moz-image-region: rect(0px,16px,16px,0px)!important;
}
