I trust that "Ctrl+T" does work though.

The only thing I can think of is corrupt localstore.rdf though that sounds different than the normal buttons can't be moved, bookmarks menu not working.

As the directions indicate take a picture of your toolbars before starting.

I would go with the manual method bring up your profile directory using Firefox then with Firefox down (verify it is down in the Windows Task Manager), then removing localstore.rdf from your profile. Then bring Firefox back up and check results.

This issue can be caused by the Ask.com toolbar (Tools > Add-ons > Extensions)

Your system details show that toolbar in the user agent as AskTbLPY/3.12.2.16749
