Firefox history does not show a lot of visits for a website that happened last year but shows a visit for the website from this year that did not happen. What can have caused this and how to fix it?

You can make the "Most Recent Visit" column visible in the Library to see when this visit happened. Note that Firefox only shows the most recent visit and hide earlier visits. You would have to use a SQLite viewer tool to inspect places.sqlite to see all the visits and the visit count from a specific URL.
