When I enable DoH, my hosts file is not respected. Is this a feature of DoH, or a bug?

Just to clarify, is this with Firefox for Android or are you using a desktop copy of Firefox?

Desktop. The behavior seems the same on Windows, OSX and Ubuntu

I’m keen to test solutions on latest Ubuntu 18 or Win10....
