No login drop down menu in firefox 59.0.2
When access "www.scanhealthplan.com" website, I click the "LOG IN" button (on the top of the page) to get the drop down menu to enter my ID and password. However, If I restarted the FireFox in safe mode with all add-on disable, I get the drop down menu when access the same website. Apparently Tracking Protection is blocking some essential content. If you have Tracking Protection enabled then you need to check for the presence of the shield icon and if necessary disable the protection for the current page. Content blockers work with filter lists or in case of Tracking Protection all content from specific domains is blocked.

I think I have identified the issue. However, I am not sure why only affect this site. Hope some senior or experienced or very technical person in the FireFox community could explain it.
Under the FireFox Preference (under Mac) > "Privacy & Security" > "History" > FireFox will use custom settings for FireFox" > select "Always use private browsing mode"
This will cause the "LOG-IN" drop down menu NOT to show up. Once I disable it (uncheck the box), the "LOG IN" drop down menu shows.
I always has this option selected since early versions of FireFox without issue. Why private mode failed this site?
PS: I never use scanhealthplan site before. This is my new health insurance. Thus, this issue might be there all along but I fail to notice before.
Furthermore, if I enable "Always" for "Use Tracking Protection to block known Trackers" under "Tracking Protection" (under "Privacy & Security") will result the same issue.
The default of this option is "Only in private windows" when FireFox was first installed.
Should it be "Always" is better to protect all users?
You always have to be aware of issues where Tracking Protection is blocking content and check if you see the shield icon at the left end of the location/address bar.
You can check the Web Console for more detail about what content is affected.
If I restore the Tracking Protection to default that is "Only in private Windows", I still have the same issue as long as I use private browsing mode. Is private browsing mode = Tracking Protection "Always" on?
Because with Tracking Protection in default and uncheck "Always use private browsing mode" under "History", my issue is gone (or resolved).
