Try to use "Firefox > Exit" (Windows: Firefox/File > Exit; Mac: Firefox > Quit Firefox; Linux: Firefox/File > Quit) to close Firefox if you are currently doing that by clicking the close X on the Firefox title bar.

On Windows XP, double-clicking the Firefox icon while Firefox already is running should usually open a new window. But does it just bring the existing window to the front (for example, if Firefox is minimized down to the Task Bar, Windows opens it back up)?
Meanwhile, consider using one of these methods to open a new window from inside Firefox:
- File menu > New Window
- Ctrl+n
- Shift+click a link
- right-click a link > Open Link in New Window
- Shift+click a bookmark
- right-click a bookmark > Open in a New Window
On the second problem, when you exit Firefox and try to start it again, do you get an error message saying Firefox is already running? Maybe something in this article will help: "Firefox is already running but is not responding" error - How to fix.
