I have downloaded Auto Fill Forms Add-In but it does not appear on my Add-In toolbar. Where is it?

I have downloaded auto Fill Forms 0.9.8.1 and set up my default profile. After saving it I could not find the icon anywhere. I have my Add-On toolbar on but the only icon on it is my Personas Interactive. I recently upgraded to Windows 7 from Vista. I used to have the Auto Fill Add On working with Vista and Firefox.

How do I get the icon on my Add On toolbar?
