Only one instance of Firefox can start.

Only the first window of Firefox starts. Further tries end with no error message, and no new window. Multiple tabs run OK, but only in the first window. It does not matter whether the second window is tried via a desktop icon or via the "Files->New Window" menu item. After closing that window, no new window can start until task manager is used to delete a process -- I think "mozilla.exe", if I remember rightly.

No such problem with Google Chrome.

Do you have that problem when running in the Firefox SafeMode? http://support.mozilla.com/en-US/kb/Safe+Mode

Don't select anything right now, just use "Continue in SafeMode."

A simple way is that, you can attach a parameter at the firefox shortcut, like this:

"C:\Program Files\Mozilla Firefox\firefox.exe" -new-tab about:blank

Then only new tabs can be openned in current FF window when you click shortcuts on desktop at least.
