Open the about:profiles page, and create a bookmark.

For choosing at startup, you can use a special shortcut. Check your All Programs in Start Menu for a Firefox folder which may contain a shortcut to start with profile manager. If not, we make the shortcut by copying a Firefox shortcut, right-clicking for it's Properties, select Shortcut tab, and add --profilemanager at the end of the path to the executable. Save this and name it as you wish.
