Print options are very limited in Thunderbird. The best way is to export the address book as a comma separated file (.csv) and then open the file in Excel. You can move the data to your liking much easier there. You have the added benefit of having the exported address book as a backup in case something goes wrong in Thunderbird and you lose the address books.
