Why is one tab just completely disappearing from the top, but is still accessible when shift-tabbing through tabs?

I've been trying to solve this problem since FF 37 and above. It's particularly hard to track with how intermittently it appears. Basically, I always close the browser through closing the last tab, which made me notice when I closed my "last" tab, I ended up on another, that didn't show up on the tab browser up top. I could open other tabs like normal, but still could shift-tab my way to the invisible tab.

I've tried disabling all extensions, select extensions, etc. with no particular headway into figuring out why this happens. Thanks for anything you guys can suggest.
