Setting Downloads to alway ask
In Tools> Options > General > Downloads I changed to Always ask me where to save files. In early Jun I downloaded a pdf and it went to where the last download went.
This function appears to be broken.
I made the change because I was not able to tell
where a file was without having to use Start > Search.
I want to be able to select the folder where the download goes.

Current Firefox versions remember the download directory based upon the URL, so if the URL changes then the default folder may be chosen if there hasn't been selected a download folder before for that server.
You can disable this feature to remember the download directory via the Boolean pref browser.download.lastDir.savePerSite on the about:config page.
The browser.download.lastDir.savePerSite pref doesn't exist by default (it is a hidden pref), so you need to create it.
Create a new Boolean pref with the name browser.download.lastDir.savePerSite and set the value to false.
You may have to clear the "Site Preferences" via Clear Recent History to remove already stored data.
The above long work around should not have to be done if the Tools> Options > General > Downloads > Always ask worked
Where do I report this non-functioning to the developers.?
Always ask should work and present with the file picker dialog to specify the name and location.
If you've set an automatic action for a MIME type then you won't get the initial dialog asking what to do.
Did you check for problems caused by an extension?
I do not know what the second sentence means. I have never to the best of my knowledge set an automatic action for a MIME type I would not know how to do that anyway (How can I copy the 6 Extension details I have?

An automatic action means that you've ticked the box in the download dialog to "Do this automatically for files like this from now on".
You can check the current setting for Adobe Reader (PDF) files in Tools > Options > Applications
