PDF's keep opening in Firefox, even though I set the Action to "Always ask" for application/pdf content type. why?

Tools -> Options -> Applications

I have Action set to: Always Ask for content type: application/pdf

When I send a valid pdf with the http header: Content-type: application/pdf

I expect to be asked if I want to download the file or open it. The file opens in the browser without asking me.

Interestingly, if I send a corrupted pdf, I get the option.

In Adobe Reader X, I have changed: Edit -> Preferences -> Internet

so that all settings regarding the web browser are unchecked (including "Display PDF in browser").
