Just as Firefox4life78 suggests just get rid of the persona add on and job done :) Thanks that was helpful :)
I just installed FF4. On XP, FF4 won't open maximized when in the "no menu mode". It opens maximized when in the "menu mode". When in the "no menu mode" I have to click the maximize icon twice to get it to fully maximize. Also the close (X) icon is not visible. I do not have a persona add on installed.
Right click the top of the window, the same bar that the tabs appear in. A drop box will appear and you will see "Menu Bar" is unchecked. Check this bar, and the buttons will appear again. This is what worked for me, I don't know if it is what you're dealing with though.

I have the same problem but i am unable to see all 3 buttons with either "menu mode" on or off. The only way it shows up is if I go fullscreen (F11). Also never used any personas or skins or anything of the sort.
-F11 on and off multiple times with restart -Toggled "Menu Bar" on and off with restart -Changed window sizes with restart
