Why are the contents of the "Most Visited" folder in "manage bookmarks" is different from its contents in the bookmarks toolbar.

In my "Manage Toolbars" window, my "Most Recent" folder shows a bookmark for my Gmail account. However, in the bookmark toolbar, the "Most Recent" folder never shows that Gmail bookmark. Why not? How can I have that bookmark displayed from the toolbar?
