I wasn't doing anything complicated or strange (like an update or anything) when I noticed it stopped working. All of a sudden I just couldn't click the plus to open another tab - nothing happened.
Also, a 'forced' tab open DOES work - like when I click on a link in facebook or something, it does open into a new tab, but if even select that option my self by right clicking, it won't respond.

My problem was solved when I un-installed the yahoo toolbar.
