If I installed a malicious add-on, what would be the process for removing it? Is removing the add-on enough? Or would you have to do a full refresh of firefox?

Have you noticed any ongoing problems?

For most extensions, removal and then cleaning up any changed settings, should take care of it.

Sometimes an extension was injected by external software, or had a companion program. To check for that, you can use malware cleaners such as the ones suggested in this support article: Troubleshoot Firefox issues caused by malware.
