on op tab bar site that i deleted and apears on restart Mozilla firefox

On top pin bar tabs site there is one "cobrahealthplan......" at the end of the pin bar there is one showing "New tab" i never installed i tried to delete it by doing the following 

1- X the sites out on the top pin bar
2- went to tools- options-General- Home Page

3- on home page deleted the tab" http://cobrahealthplan.com......" 4-ok at the bottom 

and re booted computer it still shows up with the same web page
