This happened

Every time Firefox opened

Remove a saved password here: Firefox > Preferences > Security: Passwords: "Saved Passwords" > "Show Passwords"

Remove a saved password here: Firefox > Preferences > Security: Passwords: "Saved Passwords" > "Show Passwords"

For mac users highlight the username and hit command shift delete.
