Could you check whether it is listed on the Extensions button drop-down? If you find it there, you can pin it to the main part of the toolbar:
Manage your extensions using the extensions button in the toolbar
If it is not listed, could you use the "Manage extensions" button at the bottom of the drop-down to make sure it is still installed and enabled.
