How does Firefox determine the most recent downlaod folder? Why does it change arbitrarily?

I have set the Downloads preference to "Always ask you where to save files".

When I download a file (I usually download PDF files), Firefox presents the file picker dialog with some directory pre-selected.

I would expect Firefox to pre-select the directory where I saved my most recent file.

However, Firefox often switches between several of my recent directory, seemingly absolutely arbitrarily. It would present the most recent directory e.g. 5 times in a row then suddenly switch to some other directory on its own.

This is EXTREMELY annoying and bad, as it is easy to overlook the changed directory and save the file in an unintended directory. It happened several times to me.

This behavior so destructive and annoying that I would consider it a bug that should get fixed (other browsers do not do this!). However if it is for some strange reason considered to be a feature, how can it turned off and how can Firefox be made to ALWAYS present the most recent download directory?
