Header/footer content not printing

I'm using Firefox 48.0.1 on Windows 7. I "visited" about:config, filtered by "header", and reset all values to their defaults. This includes title ("&T") on the upper left and URL ("&U") on upper right. Neither the preview nor the resulting printed PDF has this content. I have 1" margins on all 4 edges. Help?
