Open this bookmark in a tab and click the highlighted star to open the edit dialog. Click the remove button to remove all the copies. You should also be able to do this if you search for this bookmark in the sidebar (Ctrl+B) or in the Bookmarks Manager (Library; Ctrl+Shift+O).
